Carmel, NY - Music brings great joy to the world. But not without musicians paying a heavy price in occurrences of pain throughout their careers. Playing an instrument is physically demanding and practice and performing often require many hours a day in stressful positions that can cause pain, numbness, weakness, or lack of control in the wrist, hand, fingers, arm, elbow, shoulder, and neck. "Overuse injuries to the hand are a serious threat to all musicians, whether or not they play professionally," says Dr. Yariv Maghenof Somers Orthopaedic Surgery & Sports Medicine Group. "The hand is a very complex organ, with multiple bones, joints, ligaments, tendons and nerves. Repetition, fatigue, friction, vibration and awkward, static positions are all contributing factors to hand injuries in musicians."

The most common repetitive strain injuries among musicians are tendinitis and carpal tunnel syndrome. Tendinitis, the swelling or inflammation of a tendon, usually affects the wrist in musicians. Overwork under conditions of high muscle stress leads to tearing and abrasions of the tendon and pain usually made worse by repetitive motion.

Carpal tunnel syndrome is caused by compression of the median nerve that passes through the carpal tunnel in the wrist. It stems from excessively subjecting the finger and wrist flexor muscles to repetitive and/or static flexion. If the flexor tendons become inflamed, the swelling presses on the nerve causing progressive symptoms of numbness and tingling in the hand or fingers, pain, reduced dexterity and reduced grip strength.

Dr. Maghen says: "There are many more potential hand injuries that are related to playing an instrument, most of them caused by overuse, repetitive strain, poor posture and incorrect positioning of the hands, fingers and arms. Most can be treated effectively with rest and by correcting posture and positioning, especially if addressed at the first onset of symptoms. But if allowed to progress, these injuries can be physically, psychologically and even financially devastating to musicians. So preventing injury in the first place should be foremost in every musician's mind."

Dr. Maghen's tips for injury prevention apply to all musicians - professional and amateur, serious and casual, beginner and expert. They fall into two broad categories: positioning and conditioning.

Positioning

It is critical that every musician start by learning proper body position and posture. It's easier to develop good habits than to break bad ones. As a musician grows and the technical difficulty of the music increases, good technique and training are the foundation for continued ability to perform without injury. Poor training can put the musician at risk when difficult or advanced passages require perfect technique. Those who play a musical instrument as a hobby are not subject to the same pressures as professionals but still run the risk of getting hurt. When playing for pleasure and no longer taking lessons, it's easy to forget basic principles of posture, hand position, and technique. It's a good idea for the casual player to have a professional instructor observe their playing and correct any technique problems that might lead to future injuries.

Work settings should be designed to allow the musician to easily see the music as well as the conductor. Awkward positions, improper seating, poor lighting, and cramped performance spaces can cause a musician to play the instrument in an unnatural way and risk fatigue and injury.

Conditioning

A musician should prepare for practice and performance the same way as an athlete preparing for a workout or event. Before a practice session or performance, warm up with gentle stretches of the hands, arms and shoulders. This should be followed by playing simple passages of music slowly such as scales and arpeggios. Away from the instrument, a healthy lifestyle, taking into account rest, diet, exercise and overall fitness, keeps the musician in peak condition.

Demanding pieces should be practiced in chunks, taking small sections of the piece at a time, and then gradually working up to playing the piece in its entirety. Stop playing at the first sign of pain or discomfort and don't try to work through it. Taking frequent breaks and stretching can relieve the repetitive cycle of playing and allow muscles and tendons to rest and recover.

Dr. Maghen concludes that it is mostly professional and serious musicians that are at risk for injury, primarily because of the amount of time they practice and play. But he adds that it is important that all musicians listen not only to the music but to their own bodies. Proper attention to positioning and conditioning will increase the opportunity for musicians to play and focus on making beautiful music without pain or discomfort

Are you a bodybuilder, fitness competitor, power lifter, athlete, or MMA fighter?

All of these activities, recreational or competitive, have different physical demands and different nutritional requirements, which is why there is no such thing as a one size fits all nutrition plan.

For bodybuilders it is all about building lean muscle mass and maintaining or reducing body-fat depending on the training season.

Every successful bodybuilder would agree that whole foods form the foundation for a solid nutrition plan. As bodybuilders, we eat foods for what it does, the results it produces, and then how it tastes.

Once in a while we may go for taste first, but during competition season we don't have that luxury.

In this post, let's review the top ten bodybuilding foods that are protein-packed and carbohydrate-rich. These foods help pack on the muscle while staying lean in the process.

1) Almonds - Almonds are a great filling snack food for bodybuilders containing many vital nutrients, including protein and healthy unsaturated fats. About 1 oz of almonds contains 6 grams of protein, 6 grams of carbohydrates, 14 grams of fat and 163 calories.

2) Asparagus - While it's not my favorite vegetable for taste, you can't ignore the natural diuretic properties of asparagus. Asparagus helps your body maintain a balance of electrolytes, namely sodium and potassium. Bodybuilders turn to asparagus during a pre contest diet to help them get extra tight for the stage.

3) Egg Whites - A single egg white contains about six grams of high quality protein which is optimal for muscle recovery. For a bodybuilder it's not uncommon to eat 8-10 egg whites during a single meal. Six egg whites and two yolks will provide your body with 26.5 grams of protein, o carbs, and 5.23 grams of fat and only 153 calories. Personally, I prefer to nuke my eggs in the microwave because it's fast and convenient.

4) Chicken -A single 6 ounce chicken breast has close to 35 grams of protein and only 4 grams of fat. It's a great food not only for its nutritional value but because it can be prepared in so many different ways. Personally, I like to cook my chicken with crushed red peppers and dip it in barbecue sauce for a better taste.

5) Cottage Cheese - Definitely not my favorite for taste but it is a great source of casein protein. As a bodybuilder you need slowly released protein (casein) throughout the day especially when you might be without food for a long period of time.

6) Fish - Fish is the staple of most bodybuilding diets, especially during pre contest prep. Fish is a high protein type and contains no carbs, low fat and low calories. Some favorites are mahi-mahi, tuna, orange roughy, and tilapia.

7) Lean Beef - Nothing tastes better to me than a nice juicy steak. Steak is an awesome source of protein and you got to love the pure creatine you get from it too!

8) Oatmeal - Oatmeal is a favorite because of its slow burning sustainable source of carbs. Personally, I like to cut up some bananas and mix it in my oatmeal so it tastes better.

9) Sweet Potato - Slow-burning carbs such as sweet potatoes are an excellent source to fuel and sustain your muscles. Personally, I like to add cinnamon to mine.

10) Turkey - Turkey is another must have food for bodybuilding because of its high quality protein and low fat amount.

If you are serious about your training you should consider a customized nutrition plan that is based on you; your age, weight, height, metabolism, body type, training regimen, lifestyle and goals.

Despite pleas, missing SC boy falls by wayside

COLUMBIA, S.C. (AP) ? Despite detectives' pleas to national media, the disappearance of an 18-month-old black boy with the wide smile has yet to grab the widespread attention given to other missing children's cases. Some advocates say the reason why may be as simple as the toddler's gender ? and his race.

From the still-unsolved slaying of 6-year-old JonBenet Ramsey more than 15 years ago to the disappearance and killing of 2-year-old Caylee Anthony, the public has watched with rapt attention as many cases involving young children unfolded, often over many months. Yet Amir Jennings, the little boy who hasn't been seen since he was captured on surveillance video with his mother in South Carolina nearly a year ago, has registered as scarcely a blip on the nation's consciousness.

"Media has always leaned toward the cute little kids," said Monica Caison of the Wilmington, N.C.-based CUE Center for Missing Persons. "And unfortunately, a lot of times they think cute little kids are white."

Amir's mother, Zinah Jennings, was convicted Friday on a charge related to his disappearance and sentenced to 10 years in prison. The 23-year-old woman has been jailed since December, and police arrested her after she told them false, misleading stories about the boy's whereabouts. Jennings has maintained that she left the boy somewhere safe, but prosecution witnesses said the young mother claimed she was stressed and pondered selling or giving away the boy.

Jennings' mother says she last saw her wide-eyed, giggly grandson early on the morning of Nov. 28, 2011. He went to a bank with his mother the next day but has not been seen since. A store owner has testified she saw the boy and his mother a month later, but prosecutors challenged that assertion, and there was no surveillance video to back up the claim.

In the months since he disappeared, Amir's grandmother has celebrated his second birthday. His mother has given birth to a second child. And the national spotlight that initially shone on the case has waned.

One of the reasons could be as simple as Amir being a boy. While federal officials say the numbers of the missing are roughly split when it comes to gender, Caison said pedophiles tend to seek out girls, while missing boys often are taken by a parent or other relative.

And in her searches for adult males, Caison said, she has an even harder time getting anyone to pay attention.

"People want to think that missing males are OK and safe," she said. "I still sit back every day and scratch my head and say, 'Why can't you pick these cases up?'"

Amir's story has gotten nowhere near the attention of cases like that of Caylee Anthony, a 2-year-old white Orlando girl whose body was found a month after she was reported missing in 2008. Anthony's mother was arrested and charged with murder after telling a string of lies to the police.

The case captivated the nation for months and culminated with the trial of the girl's mother, Casey Anthony. Radio shows enlisted attorneys to provide analysis during the morning commute, while cable television networks covered every moment in the courtroom.

People camped outside the courthouse to make sure they could sit in the gallery the next morning. Protests erupted when Casey Anthony was acquitted of a murder charge; her attorneys devised an elaborate plan to shake the media when she was whisked away from jail.

In the Ramsey case, water-cooler speculation swirled for years about who killed the child beauty pageant queen in 1996 and who wrote the ransom note found at the murder scene. Her parents, John and Patsy Ramsey, were demonized by the public for years until prosecutors apologized and said DNA evidence excluded them as suspects. No one was ever charged in her death.

So why are some cases elevated in the public sphere, while others are not?

Jacqueline Fish, a former law officer and current criminal justice professor at Charleston Southern University, said law enforcement ideally takes each case seriously, and each case has had police and prosecutors who have spoken publicly about the need for justice. But inherently, Fish said, every case is still somewhat subjective. Columbia Police Chief Randy Scott is black, and surmised he might have seen something of one of his own children in Amir ? and pushed initially to publicize the case.

After Jennings' arrest, Scott reached out to the media to ask for help finding the missing boy. Yellow flyers began popping up around Columbia. Groups organized vigils to pray for Amir's safe return.

"I want someone to call us and say, 'We just saw this on the news, we have Amir, we're sorry, we didn't realize this was going on,'" Scott said at a January news conference announcing that a tip line had been set up. "Her stories are so across the board."

In his investigation's early days, Scott also appeared on several national cable news shows, saying that Jennings continued to change her story when pressed for information about her son. Jennings' mother also made appeals for help, asking at a news conference for any information about the boy she called "Mir Mir" and "AJ." She sat down several times with The Associated Press, describing her conflicting emotions of concern for her grandson and support for her daughter.

But as the weeks dragged on, and no credible tips moved the case forward, the national news outlets stopped calling. Scott said his officers continued their investigation, but no bombshells came.

According to the U.S. Department of Justice, about 800,000 children are reported missing in the United States a year, and nearly all reported missing to the police ? almost 99 percent ? are returned home alive. More than half of those are white, while about 150,000 are black, and 164,000 are Hispanic.

Amir's body has not been found, although police have said from the beginning that they feared foul play had been involved in his disappearance. But it's the uncertainty of his fate, Fish said, that could play a role in the lack of widespread attention.

"Someone needs to be brought to justice," Fish said. "In Amir's case, they can't be out for justice because we don't know what happened to him."

Officials with the Black and Missing Foundation, Inc., an organization that focuses on finding missing minorities, said they struggle to get and maintain news coverage of minority missing persons cases.

"We are making some headway, but there are still challenges," said co-founder Natalie Wilson, who said she sometimes gets pushback when pitching a story to media outlets.

Noting she has had some recent successes pitching missing minority cases to media outlets, Wilson said she's often told that editors and producers can't promise coverage and don't have the time to run a big piece. In one instance, a plea for help to find a young missing black girl was bumped to report the news that Paris Hilton had been released from jail.

"How does that supersede someone's life?" Wilson asked. "Can you imagine how her parents would feel?"

Attention on a missing child case should be the same ? intense ? regardless of gender or race," Caison said.

"It's not an excuse," Caison said. "A child missing should be aired because of the fact that they're a child, that they're away from safe haven, and that there's foul play or other concerns involved."

Michelaneous by Michele: Oh My Buns!

The name of the new caf? at Park Place Mall, ?O?My Buns!? stopped me in my tracks this afternoon. When I burst out laughing, my daughter didn?t know the reason for my absolute amusement other than the clever/funny name for a bakery/coffee shop.

?We?ve got to get a photo!? I said. She wasted no time whipping out her phone and snapped the shot. She then texted it to me and I, in turn, sent it to my four siblings. I knew I wouldn?t have to explain it and they, too, would also get a good laugh.

Yes, it?s an inside family joke that I?m about to let out of the oven.

Many, many years ago when a tri-level house in Brookfield, Illinois housed a family of seven?a World War II vet and his war bride, along with their five baby-boomer children?the dining room of this cozy home held family dinners every evening and Sunday afternoon/after church dinners with Grandma joining them. Actually, I don?t remember if the setting for my story was a typical Sunday afternoon or if it was something more formal or festive like a Thanksgiving dinner. I do know my mom had, as usual, taken care of the entire meal preparation.

After filling the hot tray and the Lazy Susan will all of her efforts she was the last to sit at the table. The main course had been placed in front of Daddy, who took care of carving and who set the mood of the meal. He ruled that table. No one, for example, was allowed to leave said table before Dad took his last bite.

Unfortunately for us?a group of antsy kids?he was a very slow eater.

Well into the meal that day, suddenly a look of panic covered our mother?s face. This, by the way, was a very demure woman of few words. She was quite petite and very proper, and always exhibited impeccable table manners. Something was obviously wrong.

I?m not sure if she smelled burning food or if she simply noticed an important omission on the table, but covered in panic, she stood up and shouted, ?OH MY BUNS!? Immediately, she ran from the table and into the kitchen.

Shocked, we all looked at one another as if to say, ?did that just happen?? In a moment, we all cried in unison: ?OH MY BUNS!? And we giggled with laughter through the rest of the meal. ?Even Dad thought it was very funny.

The tale of Mom?s burnt buns is one of those family stories we bring up with one another again and again. But we don?t need to go into the detail that I?ve just conveyed. ?All any of us has to say to one another is ?Oh my buns!? and it?s like our dear sweet mother is right there with us.

That?s exactly how I felt today when I saw the sign for the new caf?. It was just like a little heavenly hello.

And talk about heaven, of course, I had to buy one of their original coffee buns. It was large, cost about $2.50 and Willow and I split it. The verdict? It was the most delicious bun I?ve ever eaten. Light and flavorful, it was the tastiest treat I?ve had in a long, long time.

Palestinian PM says willing to resign over economic protests

RAMALLAH (Reuters) - Palestinian Prime Minister Salam Fayyad said on Thursday he was willing to resign if the public demanded, after three days of street protests against high living costs across the occupied West Bank, but did not say that he was going to do so.

A fuel price increase of about 5 percent sparked the demonstrations in the Palestinian cities of Ramallah, Jenin and Hebron, where protesters burned an effigy of Fayyad, who presided over economic policy as finance minister until he was replaced in that role in May.

"If there is a real public demand that I resign, and if it would solve the economic problems, then I would not hesitate to step down, nor would I delay it," Fayyad told reporters in Ramallah.

Echoing slogans chanted in Arab Spring demonstrations, the crowds called for Fayyad to "Get out," and accused the former IMF official of corruption and of leading policies harmful to the poor.

The aid-dependent Palestinian economy in the West Bank is facing a deepening financial crisis due to a drop in assistance from Western and wealthy Gulf backers, as well as Israeli restrictions on trade.

The Palestinian Authority (PA) is grappling with a recurring deficit and external debt, both of about one billion dollars, or nearly a fifth of gross domestic product, and a fifth of the West Bank's 2.5 million Palestinians are unemployed.

The PA's cash crisis delayed salary payments for some 153,000 civil servants several times this year. Fayyad said he did not know when the money would be available to pay last month's salary.

Poor planning and continued, rigid Israeli controls have led to boom time Palestinian growth rates of 9 percent in 2010 to fall by half. A recent Palestinian poll found 72 percent of West Bank respondents thought the PA institutions were corrupt.

Palestinian President Mahmoud Abbas said on Wednesday the "Palestinian Spring has begun" and that the protesters' demands to reduce costs were "right and fair." He did not voice support for Fayyad in his remarks.

Fayyad has been widely credited by the West for introducing transparency to the Palestinian economy and for his efforts to ready Palestinian institutions for future statehood.

"Despite the fact that some of the (protesters') slogans hurt me deeply, freedom of expression is guaranteed," Fayyad said.
